Hazel Dell OB/GYN provides quality care for obstetric and gynecologic patients. Our physicians offer comprehensive women’s healthcare, including office gynecology from adolescence through menopause. Our physicians perform gynecologic surgeries as well as specializing in the care of high-risk obstetrical patients, including multiple gestation, hypertension, diabetes, etc. We are able to offer expert care in a small group setting. Additionally, our group includes a surgeon who performs bladder urodynamics and focuses on vaginal prolapse and urinary incontinence treatment.
Our staff includes a nurse practitioner, a registered nurse and certified medial assistants to provide our patients with excellent care. Each of our certified medical assistants is assigned to a specific physician to ensure that every time our patients come for a visit, they see a familiar face. Also, for the convenience of our patients who require laboratory services, phlebotomy is performed on site. In addition, our office location offers the Essure sterilization procedure and in-office diagnostic hysteroscopy, ultrasound, and other clinical services.
We participate with many different PPO and HMO plans, as well as accepting traditional indemnity insurance. If you are not sure about our provider status with your insurance company, please check with your insurance company or with our business office. Plans we participate with include Aetna US Healthcare, , Anthem Blue Cross and Blue Shield, ChoiceCare, CIGNA HealthCare, Encore Health Network, Indiana Health Network (IHN), Medicare, Great West-One Health Plan, PPOM, Private Health Care Systems (PHCS) PPO, Prudential Healthcare, Sagamore Health Network, St. Vincent Care Management Organization (CMO), and United Healthcare.
